The following information has been updated: Call for nominations The National Coordinating Center for the Genetic Service Collaboratives needs help identifying individuals and/or clinics/programs that demonstrate exemplary practices related to coordinating care for children with genetic conditions, particularly complex conditions, and their families. See the nomination form (Word) and (PDF version) for additional details. The NCC will contact selected nominees to learn about how they developed, implemented, and sustain these practices, with the aim of compiling and publishing descriptions of them, key lessons learned by the nominees, resources found useful, and contacts for more information. You can use the form to nominate them by return email or you can do so through SurveyMonkey at https://www.surveymonkey.com/s/HRLX723. This is a project of the Medical Home Workgroup of the National Coordination Center for the Regional Genetics Collaboratives (NCC) funded by the Health Resources and Services Administration (HRSA), led by the American College of Medical Genetics and Genomics (ACMG), and collaborating with the Genetic Alliance and the American Academy of Pediatrics. The goal of the Workgroup is to promote, advance, and spread the medical home model of care for children with heritable conditions by sharing knowledge of approaches that work and connecting individuals and groups with similar interests.
